COVID QUESTIONAIRE
We now have the ability to complete our COVID Questionaires in an online form. We will have QR code stickers at the club, outside the door, and on the sandwich board so that you can complete these ahead of time. Paper copies can still be completed if no access to the link. All participants (or guardians) must complete a questionaire before participating.
